Her progress is steady and pronounced. Visual acuity has improved substantially, although she is still visually impaired and likely always will be. The greatest improvement has been in her quality of life; she plays with the young dogs, eagerly begs for daily walks and shows us in a hundred little ways every day how much happier she is.

RECAP

Jessie is now almost 12 years old and has suffered from severe, chronic dry eye since approximately age 2. We tried many treatments over the years until finally finding a new vet with experience in treating dogs like Jessie. His medical treatment, combined with my use of human, over-the-counter dry eye products and fish oil supplements has lead to a slow but steady improvement in Jessie's condition.

UPDATED TREATMENT PROTOCOL

Jessie was diagnosed with arthritis of the spine 3 months ago and has been taking 5 mg of Prednisone three times per week for relief. The constant level of steroid in her system has also been a benefit to her eyes, so we have stopped using the implanted steroid 'blebs'. She continues to take 4000 mg of fish oil per day, divided into two doses. Her eyes have progressed so well that they now require just two cleaning/lubricating sessions on most days. I still flush any debris and accumulated mucus from her eyes with the Eyes So Bright eye wash, and follow it with two drops per eye of the Neo/Poly/Dex drops (combination steroid and antibiotic). I apply the GenTeal Severe Dry Eye lubricant in the morning and the GenTeal PM Severe Dry Eye ointment at night.

SUMMARY

I can't stress enough the importance of each step in Jessie's treatment protocol. As you will see from the updated photos to follow, her eyes, though still far from 'normal' are much improved, as is her coat. The systemic lubrication of daily fish oil has done wonders for her fur, pads, nose and eyes. I purchase ordinary, human supplement fish oil from the local drug store, but caution is advised when selecting a fish oil product. Some contain 'filler' ingredients that have no benefit whatsoever and may even inhibit the effectiveness of the fish oil. READ LABELS. Purchase only capsules of pure fish oils without additives!

I still highly recommend Eyes So Bright for all dog owners. This product should be in your canine first aid kit. It not only flushes foreign matter from the eyes but contains soothing ingredients for improved eye comfort as well. The GenTeal product line has been a true life-saver for Jessie and we will continue to use several of their products in her treatment.

PHOTOS

Close-up of Jessie's Left Eye. Other than that, there was nothing remarkable about any of the pups at birth. Everything seemed normal.

The puppies grew and developed in ordinary fashion for the first few weeks; I saw nothing to cause any particular concern. It wasn't until they began to sit up, stand, and then walk, that I realized I had a pup with a problem.

At nearly four weeks old, Fudge was unable to stand, let alone walk. All the other puppies were walking, playing, fighting and generally behaving like puppies always do. Fudge, on the other hand, was not doing any of these things. He laid flat on his belly at all times, with all four legs splayed out to the sides. When he moved to his mother to nurse, he looked like a soldier crawling under concertina wire, scooting along on his tummy.

When I held him, I could feel how flat his rib cage was; he had trouble breathing. I made an appointment with our vet to confirm my ‘diagnosis’ of Swimmer Puppy Syndrome. At the time, I had never seen a case in person.

The vet seemed to think I was overreacting; he assured me Fudge would outgrow it. I refused to accept that, since the poor baby was already four weeks old and having respiratory problems. A spin around the Internet wasn’t very encouraging, either. Most sites I found recommended euthanizing afflicted pups to prevent them from dying a slow and painful death. I looked this sweet little pup in the eyes and knew that was not a viable option, so I kept searching.

Thank God for Coreen at Oak Ridge Cockers, a Cocker Spaniel breeder with experience saving Swimmer puppies. Her site was full of information and positive encouragement, and gave me the basis for Fudge’s treatment that ultimately saved his life. If you visit her site, you will see a synopsis of Fudge’s story.

Treatment

I didn't think to take photos of "Fudge" before beginning to work with him, but there are several taken early on in his therapy. The first priority is to get the pup off it’s chest so it can breathe more normally. I adapted the information from Coreen to make it applicable to a much larger pup. Using an old, ribbed-knit t-shirt, I cut a large piece and made holes for Fudge’s legs. I used a thick make-up sponge under his belly, and smaller, wedge-shaped ones on the sides of his front legs. Then I tied the ends of the shirt up and over his back to hold the whole rig in place. This relieved some of the pressure on his rib cage and he was able to breathe easier immediately.

The second concern is positioning the front legs properly; the wedge-shaped sponges helped accomplish this. The elbows must be propped in tight to the body so they can bear some of the pup’s weight and get it accustomed to being in a more upright posture. This also permits you to lay the pup on it’s side...Swimmer puppies do not ever lay on their sides like normal pups do. It took three days in the ‘rig’ for Fudge to lay over on his side by himself—a major milestone.

To transport him to a follow-up appointment with the vet, I laid him in a basket. I noticed that the size and shape of the basket forced him to lay on his side and use his front legs when turning over, so I used the basket every day - a sort of 'hobble by basket' technique that seemed to help strengthen his little legs. He had not tried to push himself up and back onto his butt before the basket therapy.

Fudge’s front legs were completely rigid; the muscles were constantly tensed. I held him on my lap every night and massaged each side until he relaxed. He would sleep so soundly during this procedure that I could flip him over and rub the second side without waking him. We’re talking one blissed-out pup!

On Wednesday, August 3, 2005 I put Fudge in his 'rig' for the first time. On Sunday, August 7, 2005 he took his first steps. By August 12, 2005, I tried to photograph him walking, but it wasn't easy! All he really wanted to do was RUN! The photo in the home page header is of Fudge's little butt as he ran through my yard on August 12th.

Outcome

It took another two weeks for Fudge to fully catch up with his litter mates. At eight weeks, when he went to live with his new family, no one would ever have known he started off with problems. He's a happy, healthy adult dog with a full life expectancy. I sold him with the caveat that he not be bred, because some 'experts' believe there 'may' be a genetic component to the syndrome.

If you think you may have a Swimmer puppy, do not wait to intervene! The sooner you act, the easier it will be for the pup to catch up with normal development. I encourage you to read the information on Coreen's web site. Jack Vanderwyk has a scholarly article on the syndrome at labradornet.com. Please don't hesitate to post a comment here asking for help if you need it.

Jessie is an 11-year-old female, pure-bred, Chocolate Labrador Retriever. She first began developing eye problems around the age of two, and they grew progressively worse through the years. The underlying condition also created a tendency to frequent, serious eye infections and a chronic, thick, mucus discharge. Concurrently, she developed an extremely dry coat, paw pads and nose.

History of Treatment

I began seeking veterinary care for Jessie's eyes when she was 2 years old. The first symptom was a prolapsed third eyelid. Dogs actually have three eyelids, two you can always see and a third that normally remains hidden in the corner of the eye. It is supposed to protect the eye from debris and injury. Jessie's third eyelid was extended up onto the eyeball and remained visible at all times.

We live in a rural area, and our country vet did all he could for her, prescribing assorted antibiotics for the infections. These did nothing to help with the dryness, however, and it was getting worse.

We were finally referred to a canine Ophthalmologist. I was unaware there even was such a thing, but in our area of Michigan there is a clinic offering vets in a variety of specialties, including the eyes.

The specialist tested her eyes for tear production and found almost none. He prescribed a custom-compounded eye drop containing cyclosporine, and we waited to see if there was improvement. There was little change.

Next he took samples from the tissues of the lids and sent them to a lab for culturing. He also sent copies of her file to friends at several prominent veterinary schools around the country, asking for their input and advice. Unfortunately, the test results were inconclusive and the vet schools felt he was pursuing the only course of treatment available.

At the same time, I also began using a series of eye wash products to keep the mucus flushed out so Jessie could see. It would get so heavy that it coated the entire eye, which looked horrible and must have felt worse. I also eventually began experimenting with human over-the-counter dry eye drops.

In 2007, we were introduced to a new vet practice in our area and I began having the dogs treated there. The new doctor had seen cases of severe dry eye like Jessie's, and offered a few options for treatment, including the possibility of surgery - but he did not recommend it except as a last resort because of the complications. He prescribed steroid drops, Prednisolone Acetate, and a combination drop of Neomycin, Polymyxin B and Dexamethasone.

We used the antibiotic-based drops when an infection was present and the plain steroid all other times. I also discovered a wonderful, all natural eye wash called Eyes So Bright, which works to dissolve the accumulated mucus immediately, enabling me to keep Jessie's eyes clean. I settled on the line of human eye lubricants from Novartis called GenTeal. The extra-thick, gel products for severe dry eye relief have been a God-send for Jessie!

The combination of these treatments was keeping Jessie comfortable, but years of suffering had caused a network of blood vessels to grow over the surface of her eyeballs, leaving her almost completely blind. I was prepared to be her 'seeing eye person' for the rest of her life. My only desire was that she spend her golden years as comfortably as possible.

The new vet then recommended we try inserting what he calls 'blebs' of steroid into the tissue of the upper eyelid, allowing for continuous, slow release of the medicine over a three-month period. I agreed, and he numbed her eyes and placed the small, white 'blebs' in place with a needle. By the next morning, I could see marked improvement in the inflammation of her eyes.

About half-way through the three months, I was performing our morning eye-cleaning ritual when I noticed that I could see the brown pupil of Jessie's right eye again. It had been years since I could look in my girl's eyes and "see her". I cried.

The 'blebs' cannot be repeated continuously, so we are using the drops for now and may insert more 'blebs' in the future. The routine of flushing, medicating and moisturizing her eyes 4-5 times per day has been keeping them much healthier, and Jessie happier. None of the treatments we tried over the past 8 years has ever addressed the other dryness issues she seemed to have, though. I asked every vet who treated her if she might have some systemic problem, such as a lipid imbalance, contributing to her eye problems. None was very responsive - I am NOT a veterinarian, after all.

Recently, another of our dogs developed a condition which was causing her to go bald. Look for details on her diagnosis and treatment in another post soon. Part of the recommended treatment for her was 1000 mg of fish oil per meal, to help restore the luster and sheen of her coat. Figuring it couldn't do any harm, and might actually help Jessie with her dry, brittle, discolored coat and crusty nose and paw pads, I put her on a mega-dose of 4000 mg of fish oil daily.

I had been supplementing her food with organic extra-virgin olive oil and organic flax seed oil for several years, but our vet stressed the importance of animal fats in their diets, so I bought fish oil capsules. They are regular, at-the-corner-drugstore, made for human consumption, fish oil supplements. They are also a very inexpensive treatment.

In just one week, the changes in Jessie's skin, coat, pads and nose have been nothing short of remarkable. I wish I had photographed her before starting the fish oil treatment, but I didn't. The photos below show her as she is now, in the middle of May, 2008.

I expected to see some improvement in her coat, but the most amazing benefit of all to date has been in her eyes! The discharge has been reduced to almost nothing, she can open her eyes and see first thing in the morning without drops, and they require much less frequent use of the moisturizing gel.

The redness of the third eyelids is diminishing, and her eyes look so much healthier and improved that I cried again. So far, the improvement has been near miraculous.

I will continue to update this post with progress reports. Hopefully, Jessie will continue to improve, but even if this is as good as we can get, it is allowing my sweet old woman to enjoy the last years of her life. That's all I ever hoped and prayed for.

Current Treatment Protocol

Jessie's eyes are flushed out with Eyes So Bright Eye Wash as often as needed, usually 3-4 times per day. Then I administer steroid drops at least 3 times daily, substituting the antibiotic / steroid combo drop as needed.

For moisture, I use the GenTeal Severe Dry Eye Relief Gel at least 3 times per day, or as often needed. Before bed, I use the GenTeal PM Severe Dry Eye Relief Gel. It is specially formulated to lubricate throughout the night.

I am currently administering 4000 mg of fish oil per day, divided into two doses. I put two 1000 mg capsules in Jessie's breakfast, and two more in her dinner.

SEE UPDATE POSTED DECEMBER 6, 2008.

Photographs

Jessie's eyes.
